Short-Term Medication Form
A medication form that parents will need to complete if a student is required to take a short-term course of medicine (such as antibiotics) during the school day, and which is to be administered by the school staff.
This is a Health and Safety requirement and the school will not be able to administer medicines to students without a completed and signed copy of the form. Once completed and signed, please bring the form and medication to the school office.
